This is the most vital component on a highly effective lead capture page. The lead capture form can be best described as a graphical table in which your page visitors can enter their contact information. The form is important to have because that’s the very purpose of your lead capture page – to obtain details so that you can follow-up.
You have more than likely seen a variety of these forms. Many are made up of graphics (on webpages), but they can also be tangible. Think of the last time you went to a shopping mall, and you saw a brand new vehicle parked inside the mall. In close proximity, there was probably a ballot accompanied by paper forms. The company involved is offering the public a chance to win the vehicle.
First though, they want you (the prospect) to leave some contact information on a paper version of the lead capture form. This is often advertised as a drawing; they are capturing leads. Likewise, on your own lead capture page, you must offer something in exchange for contact information. Observe the following form.

Effective lead capture forms have labels prominently displayed. This is the common term used for the title of each box. Depending on your campaign, you can collect whatever information you want. At the very least (since you’re email marketing) you want to collect an email address. Often times, the button is easily seen too. On this button you should have a call to action. Better examples include:
